What is a Care Manager?

A Care Manager is a health and human services professional with a specialized focus on issues related to aging, elder care and managing complex, chronic care needs. Care Managers work privately with older or disabled adults and their families to create a plan of care that very specifically addresses the needs of the client. Regular follow-up establishes a sound relationship that ensures concerns are addressed and needs are met.

Care Managers are:
  • Experienced advocates
  • Skilled healthcare professionals
  • Managers of complex situations
  • Resourceful solution finders

Care for older or disabled adults
Our Care Managers are expertly prepared to set up services that manage chronic care needs. We conduct in-depth assessments, set up a customized plan for each individual, implement the plan and monitor our client's ongoing care.

Care for chronically ill children
Our Care Managers provide information and support that helps ease a family's concerns about education, growth and development, managing fragile health conditions, and promoting quality of life for their loved ones.

Sound Options is a founding member of the National Association of Geriatric Care Managers (GCM)
